I desire to have my child and/or myself engage voluntarily in the Performance System LLC exercise program in order to attempt to improve my physical fitness. I understand that the activities are designed to place a gradually increasing workload on the cardio respiratory and skeletal/muscular systems and thereby attempt to improve their function. The reaction of the cardio respiratory and skeletal/muscular systems cannot be predicted with complete accuracy. These changes might include abnormalities of blood pressure and heart rate.

I understand that the purpose of the exercise program is to develop and maintain cardio respiratory fitness, body composition, flexibility, muscular strength and endurance. A specific exercise plan will be given, based on needs, interest and doctor's recommendations. All exercise programs include warm-up, exercise at target heart rate or rating of perceived exertion and cool down. Programs may include but not be limited to walking, jogging, swimming, or cycling (outdoor or stationary), participation in exercise fitness tests, aerobics classes or choreographed fitness classes, calisthenics, plyometrics and/or strength training. All programs are designed to place gradually increasing workloads on the body in order to improve overall fitness. The rate of progression is regulated by exercise target heart rate and perceived effort of exercise.

In signing this consent form, I affirm that I have read the form in its entirety and that I understand the nature of the exercise program. I also affirm that my questions regarding the exercise program have been answered to my satisfaction. I understand that I am responsible for monitoring my own condition throughout the exercise program and should any unusual symptoms occur, I will cease my participation and inform the instructor of my symptoms.

I hereby certify that I am and/or my child is covered by his/her/my own medical insurance. I have consulted my physician before participation in this exercise program and have his/her approval. If in the event a medical clearance must be obtained prior to my participation in the exercise program or fitness assessment I will allow the Performance System LLC staff to consult with my physician and obtain written permission prior to the commencement of any exercise program. I certify that I am in good health and if that condition changes, I will notify the Performance System LLC staff. Also, in consideration for being allowed to participate in this exercise program, I agree to assume any and all risks of such exercises, and further agree to hold harmless Performance System LLC dba Golf 18 Fitness their principals, their employees, contractors, affiliates, sponsors, representatives, and volunteers, all of whom are herinafter collectively referred to as “the Releasees”, conducting the program from any and all claims, suits, losses or related cause of action for damages incurred during or arising in any way from the exercise program.

The undersigned agrees to release the Releasees from any and all liability for any loss, damage, injury, death, medical or other expense that I or my child may suffer or that any other party may suffer as a result of my or my child’s participation in the Performance System LLC dba Golf 18 Fitness exercise program. The undersigned agrees to hold harmless and indemnify from any and all liability, claims and causes of action resulting from or pertaining to the undersigns' participation in Performance System LLC’s exercise program.

I also expressly grant Performance System LLC, and any third party authorized by them, the right to film, photograph, and make any reproductions of my or my child’s physical likeness, and the irrevocable right to use and display, such likeness in any media, including, but not limited to, the exhibition, online use, print, cable, published articles in which such likeness may be printed, used or incorporated, and in the advertising and publicizing of Performance System LLC dba Golf 18 Fitness.

I am acting on behalf of my child’s other parent in signing this contract and I have authority to bind such other parent to the terms and conditions of this contract on his or her behalf.
